• In a patient with multitrauma, remember to document times so that your note reflects that there were no delays. This will protect you if the patient ultimately has a bad outcome upstairs. • Consider documenting measurement of calves 10 cm distal to the tibial tuberosity as true objective evidence on the chart that consideration of DVT/PE were considered, however not supported on examination • Avoid listing the differential diagnosis in the MDM of discharged patients unless you have pursed a workup to exclude all the listed diagnostic possibilities before time of discharge
