• Particularly in Afro-Carribean women, there can be persistence of juvenile t wave pattern till age 40s (Flipped t-waves in V1-V3) • Don’t worry about the risk of cardioembolic event when electrically cardioverting atrial fibrillation in a patient with WPW. Why? Because these patients don’t have chronic atrial fibrillation. If they did, they would be dead. So remember electrical cardioversion even when the patient is not anticoagulated is safe. Also remember that procainamide is safe. Other AV nodal blockers including amiodarone are NOT safe. • Clumps of beats, I.e. regularly irregular rhythm could represent possible PAC pattern such as bigeminy or trigeminy. OR think about second degree AV blocks. Remember that you can have Mobitz block with atrial tachycardia, resulting in overall tachycardic rhythm rather than classically bradycardic.
