• Hyponatremic patient with evidence of seizure or coma? Give 150mL 3% bolus over 5 minutes, and repeat serum sodium OR you can use the rule of 3’s: 3mL/kg of 3% over 30 minutes. Goal is to raise sodium by 3. • For chronic hyponatremia: rule of 6’s: Correct no faster than 6 mEq/mL per day, with 6mEq in 6 hours on the first day if symptoms are severe and then stop (no more correction that first 24 hours) • If you mess up, or overcorrect, “turn off the kidneys”, I.e. give DDAVP/desmopressin (antidiuretic hormone) • Most of the time, hypercalcemic patients are volume down
