• PTA pus is more commonly in the superior pole so aim superior-medially to hit the abscess and avoid the posterior-lateral placed vasculature. • Streptococcus pyogenes (gram positive aerobe) and fusobacterium necrophorum (gram negative anaerobe) are equally likely to cause PTA so choose your antibiotics carefully. • Get the imaging as it often changes management as the size of the abscess can be fully assessed. • If using an endocavitary ultrasound probe to examine a possible PTA anesthetize with hurricane spray or atomized 4% lidocaine and point probe marker toward the ear. • Admit patients who are septic, can’t tolerate PO, immunocompromised.
